Keeping You Informed Before Your Initial Dental Appointment

Waiting room at Perez Family Dentistry

Welcome to Perez Family Dentistry! Our team is ready to welcome new dental patients and is eager to meet you and your family. As a family-owned and operated dental practice, we are committed to building relationships, creating healthier smiles, and keeping patients happy. If you are ready to establish yourself as a new patient in our dental office, we invite you to review the information below and call us if you have any questions.

Your First Visit

The initial visit to Perez Family Dentistry includes filling out the necessary paperwork before one of our hygienists walks you through the office. Taking digital X-rays of your smile, Dr. Perez will then examine your smile to check for any problem areas. A thorough teeth cleaning will then be completed before receiving a complete treatment plan should you require any additional dental care in the future. We’ll then escort you to the front desk to check out.

Patient Forms

When arriving for your appointment, you will be required to fill out new patient paperwork, as this will allow us to learn more about you and any conditions or health concerns you might have. The mouth and body are closely connected, so this information will help to keep you safe when undergoing certain treatments.

Dental Insurance

As an in-network provider, we make it easy to use your dental insurance benefits. Our team works directly with your insurance company to help you get the most from your coverage and keep your out-of-pocket costs as low as possible. We’ll verify your benefits, file your claims, and help answer any questions you may have along the way. While we always consider your insurance coverage, our goal is to recommend the treatment that best supports your oral health and helps you maintain a healthy smile for years to come.
