Your First Visit
The initial visit to Perez Family Dentistry includes filling out the necessary paperwork before one of our hygienists walks you through the office. Taking digital X-rays of your smile, Dr. Perez will then examine your smile to check for any problem areas. A thorough teeth cleaning will then be completed before receiving a complete treatment plan should you require any additional dental care in the future. We’ll then escort you to the front desk to check out.
Patient Forms
When arriving for your appointment, you will be required to fill out new patient paperwork, as this will allow us to learn more about you and any conditions or health concerns you might have. The mouth and body are closely connected, so this information will help to keep you safe when undergoing certain treatments.
